Scheduling Appointments
How to Schedule
Online Scheduling:

- Select "Schedule Appointment"
- Choose provider, appointment type, and preferred date/time
- Receive confirmation email with appointment details

- Provide your name, date of birth, and reason for visit

Walk-In Appointments:
- Limited walk-in availability for urgent concerns

- Extended wait times may occur for walk-in visits
Appointment Types
- New Patient Visits: Comprehensive initial evaluation (allow 60-90 minutes)
- Follow-Up Visits: Continuing care appointments (30-45 minutes)

Requesting Medical Records
For Your Personal Use:
- Access most records instantly through the patient portal
- Request complete records by completing Medical Records Request Form
- First copy provided free of charge; fees may apply for additional copies
- Allow 5-7 business days for processing
Transferring Records to Another Provider:
- Complete authorization form with receiving provider information
- Records sent securely via fax, mail, or electronic health record exchange
- No charge for records sent directly to another healthcare provider
Visitor Information
Visiting Hours
General visiting hours vary by department. Most areas allow:
- Inpatient units: 10:00 AM - 8:00 PM daily
- ICU and special care units: Limited hours and number of visitors
- Emergency Department: Two visitors per patient
Visiting policies may be modified based on patient condition or public health circumstances.
